For nonmajor bleeding (defined as any bleeding not classified as major) that does not require hospitalization or supportive care, holding vitamin K antagonists and possible administration of vitamin K is recommended depending on INR. The 2019 National Comprehensive Cancer Network guidelines on cancer-associated VTE include rivaroxaban as a monotherapy option.34 Apixaban is included as an acceptable alternative for patients who refuse LMWH or who have compelling reasons to avoid LMWH.35.
